Telehealth Consent

As part of your clinical enrollment through Truventa Medical, you will be required to review and acknowledge the OpenLoop Healthcare Partners Telehealth Consent Form. This form provides detailed information about:

  • Your rights as a telehealth patient
  • The provider group entities that deliver your care
  • Treatment-specific consent, including compounded medications
  • Use of AI technologies in telehealth encounters
  • Authorization to bill insurance and assignment of benefits
  • Consent to communication via phone, text, or email
  • State-specific disclosures and complaint filing information

State-Specific Disclosure Requirements

Certain states — including California, Massachusetts, Rhode Island, and Vermont — require disclosure of provider group information. In compliance with these requirements:

  • California (Cal. Bus. & Prof. Code § 680.5): Provider information is disclosed to patients during the clinical encounter and is available on this page and through the Telehealth Consent Form.
  • Massachusetts (Board of Registration in Medicine Policy 15‑05): Provider identifying information is disclosed to patients through the patient portal and during the clinical encounter.
  • Rhode Island (Board of Medical Licensure and Discipline Telemedicine Guidelines): Relevant provider information is disclosed within the medical practice site (patient portal) and during the clinical encounter.
  • Vermont (Board of Medical Practice Policy on Telemedicine): Provider identity, location, and applicable credentials are disclosed and validated to establish the provider-patient relationship during the clinical encounter.

What is Telehealth?

Telehealth involves the delivery of health care services using electronic communications, information technology, or other means between a licensed health care practitioner and a patient who are not in the same physical location. Telehealth enables diagnosis, consultation, treatment, education, care management, and self-management of a patient's healthcare.

How It Works at Truventa Medical

Truventa Medical connects patients with independent licensed healthcare providers through OpenLoop Healthcare Partners, PC and its affiliated professional corporations. Consultations are conducted asynchronously (online questionnaire reviewed by a provider) or via secure video when required by state law or clinical necessity.

Informed Consent

By completing an intake form through Truventa Medical, you acknowledge and agree that:

  • Telehealth consultations are not a replacement for emergency care
  • You have the right to refuse or withdraw consent at any time
  • Technical failures may interrupt consultations
  • Your information will be handled in accordance with HIPAA
  • A provider may determine telehealth is not appropriate for your situation
  • No specific results are guaranteed from telehealth services
  • In some cases, your provider may be a nurse practitioner or physician assistant rather than a physician

Compounded Medications

If your provider prescribes a compounded medication, please be aware that:

  • The FDA does not approve or review compounded products for safety, effectiveness, or quality
  • Compounding pharmacies must adhere to strict quality control standards and are licensed pharmacies subject to state and federal regulations
  • These statements have not been evaluated by the FDA. These products are not intended to diagnose, treat, cure, or prevent any disease.
