These Terms apply to all visitors, users, patients, and any other individuals who access or use the Site. We reserve the right to modify these Terms at any time. Your continued use of the Site following any modification constitutes your acceptance of the updated Terms.
Truventa Medical is a telehealth platform that connects patients with licensed healthcare providers across the United States. We operate in all 50 states through our network of licensed healthcare professionals who practice in full compliance with applicable state and federal laws and regulations.
All clinical services — including medical consultations, diagnoses, prescriptions, and ongoing patient care management — are delivered through OpenLoop Health, a telehealth infrastructure platform that employs or contracts with licensed physicians, nurse practitioners, and other healthcare providers. OpenLoop Health maintains full regulatory compliance, including HIPAA requirements, state medical practice acts, and applicable telehealth regulations.

You must be at least 18 years of age. Our Services are not intended for, directed at, or available to persons under the age of 18. By using the Site, you represent and warrant that you are 18 years of age or older. If we become aware that a user is under 18, we will immediately terminate their access to our Services.
You must be a legal resident of the United States to access our clinical services. You must reside in a state where our affiliated healthcare providers are licensed to practice medicine. Some services may not be available in all states due to varying state regulations.
You agree to provide accurate, current, and complete information when completing any intake assessment or registration form. Providing false or misleading information — particularly regarding your medical history, medications, or identity — may result in inappropriate medical care and will result in immediate termination of your access to our Services. You agree to promptly update any information you have provided to keep it accurate and current.
You may maintain only one account and one active patient relationship through our platform. Creating multiple accounts to circumvent restrictions is prohibited.
You may not use our Services if you have previously been removed from our platform for any reason, including violations of these Terms or fraudulent activity.
Truventa Medical provides a platform through which you can access the following services (subject to provider review and clinical appropriateness):
Our digital intake forms collect general health information to help determine whether our clinical programs may be appropriate for you. These assessments are not medical consultations and do not establish a physician-patient relationship. They are a preliminary screening step designed to connect you with the right care program.
Upon completion of your intake assessment, you may be connected with a licensed physician or nurse practitioner for a telehealth consultation conducted via secure video, phone, or asynchronous messaging. These consultations establish a formal physician-patient relationship and are governed by applicable state laws and medical standards of care.
Where clinically appropriate, and at the sole discretion of the treating physician, prescription medications may be issued as part of your care plan. All prescriptions are dispensed by licensed, FDA-regulated pharmacies and shipped directly to you. See Section 6 for detailed terms regarding prescription medications.
Our platform supports ongoing care relationships, including follow-up consultations, lab review, treatment adjustments, and secure provider messaging, as part of your active subscription or care plan.
We provide informational articles, videos, and resources on topics related to men's and women's health, weight management, hormone optimization, and other subjects relevant to our treatment programs. This content is for educational purposes only and does not constitute medical advice.
All prescriptions are issued solely at the discretion of the licensed physician or nurse practitioner conducting your consultation. Truventa Medical does not guarantee that a prescription will be issued following a consultation. The treating provider has the absolute authority to determine whether a prescription is medically appropriate based on their clinical evaluation of your individual health status.
Physicians may, in their professional medical judgment, prescribe medications for off-label uses (i.e., uses not specifically approved by the FDA) when supported by clinical evidence and consistent with the standard of care. You will be informed of any off-label use as part of the informed consent process during your consultation. Off-label prescribing is a well-established and legal medical practice.
FDA Disclaimer: Compounded medications have not been evaluated by the Food and Drug Administration for safety, efficacy, or quality. Compounded medications are legally prepared by licensed pharmacies and prescribed by licensed providers when commercially available alternatives are not clinically appropriate for a specific patient.
You agree that all medications prescribed through our platform are for your personal use only. You must not share, sell, transfer, or otherwise distribute any prescription medications to any other person. Doing so may constitute a federal crime and will result in immediate termination from our platform.
You agree to take all medications exactly as prescribed, to report any adverse effects promptly to Your licensed care provider, and to disclose all other medications, supplements, and health conditions before starting any new treatment.
Truventa Medical is not acting as a pharmacy, nor does Truventa Medical control or interfere with any pharmacy services. By using our Services, you understand and agree that you may be entering into a relationship with a pharmacy, pharmacist, and/or pharmacy group or other such third-party entity.

Truventa Medical currently offers services in all 50 states plus Washington D.C. Some services may not be available in all 50 states or Washington D.C. Subject to change.
Availability of specific treatments, medications, and healthcare services may vary by state due to differences in state medical practice laws, telehealth regulations, pharmacy licensing requirements, and controlled substance prescribing rules. Your licensed provider will confirm service availability for your location during your consultation.
Your membership in the subscription program ("Subscription Service"), including your ability to access the services, begins when you have completed the intake form at which time you will be charged for the first month of the Subscription Services.
The Subscription Services include access to providers, medications where appropriate, and online support.
Please note if you are disqualified by our medical provider for any medical reason you will receive a full refund.
There is no guarantee of results or effectiveness due to your participation in the Subscription Services.
Your Subscription Services represents a single price for the subscription purchased. This may include consultations both live and via electronic form submissions and reviews with a Provider for medications that are fulfilled through a pharmacy or other health care services on which we collect payments, lab services, care support, 24/7 patient support line and other services to support your health journey. Services, labs and medications covered with the single Subscription Service price may vary.
By enrolling in a Subscription Service, you authorize Truventa Medical to charge your chosen payment method (credit card, debit card, or other accepted payment method) at the applicable rate on a recurring basis (monthly or as otherwise specified at the time of enrollment). You agree to keep your payment information current and accurate. Your subscription will automatically renew at the end of each billing period unless you cancel before the next renewal date.
You may cancel your Subscription Service at any time for any reason. You may cancel your subscription at any time either by sending an email with your name and email address associated with the service to support@truventamedical.com or contacting your provider directly.
To ensure a smooth process, we require your cancellation request to be received at least 72 hours prior to your billing date. If this notice date is not met, your subscription will be charged on the next billing date, and the cancellation will take effect on the subsequent billing date.
After cancellation, you will continue to receive the Subscription Services through the end of your billing cycle.
If your Subscription Service is cancelled due to disqualification by our medical provider, you will be issued a refund for the remainder of your Subscription Service charges following the disqualification.
Other than cancellation due to medical disqualification, IN NO EVENT SHALL YOU BE ISSUED A REFUND UPON CANCELLATION OF THE SUBSCRIPTION SERVICES.
Federal law generally prohibits the return of prescription medications to pharmacies for refund or reuse once they have been ordered to a patient in order to protect public health and safety. This policy ensures that all medications provided are handled and dispensed with utmost care and compliance. If approved, refunds will only be issued for your most recent billing cycle and will not be issued for past billing cycles.
Please inspect your medication immediately upon receipt. If the medication appears to be damaged or incorrect, please contact customer service immediately.
If your Provider determines you are no longer eligible for the Subscription Services, your subscription will be canceled, and a refund may be issued. If you have questions or concerns about an eligibility-initiated cancellation, please contact our customer service.
We reserve the right to adjust subscription pricing at any time. We will provide at least 30 days' advance notice of any price increase via email. Your continued enrollment after the effective date of a price change constitutes acceptance of the new pricing.

In all events you are responsible for all fees from services and healthcare Services rendered; this Subscription Services Cancellation and Refund Policy governs fees for Subscription Services only.
All telehealth services accessed through Truventa Medical are provided by licensed healthcare providers through OpenLoop Healthcare Partners, PC and its affiliated entities. As part of your clinical enrollment, you will be required to review and acknowledge the OpenLoop Healthcare Partners Telehealth Consent Form.

By proceeding with a consultation through the Truventa Medical platform, you acknowledge that you will be asked to review and accept this Telehealth Consent Form as part of the intake process. Healthcare services will not be provided until this consent has been obtained.
IF YOU BELIEVE YOU ARE HAVING A MEDICAL EMERGENCY, IMMEDIATELY CALL YOUR LOCAL MEDICAL PROVIDER OR DIAL 9-1-1. Telehealth services are contemplated only for specific, non-emergency medical conditions and concerns. Our providers do not address medical emergencies via telehealth.
All content on the Site — including but not limited to text, graphics, logos, images, audio clips, video clips, data compilations, software, and the underlying code and architecture — is the property of Truventa Medical or its content suppliers and is protected by United States and international intellectual property laws, including copyright, trademark, and trade secret laws.
Truventa Medical grants you a limited, non-exclusive, non-transferable, revocable license to access and use the Site for your personal, non-commercial purposes. This license does not include the right to:

In no event shall our total cumulative liability to you for all claims arising out of or relating to these Terms or your use of the Services exceed the total amount paid by you to Truventa Medical in the twelve (12) months immediately preceding the event giving rise to the claim.
The Site may contain links to third-party websites or resources. We are not responsible for the content, accuracy, or availability of any third-party sites, and the inclusion of any link does not imply our endorsement or affiliation with that site.
To the fullest extent permitted by applicable law, you agree to indemnify, defend, and hold harmless Truventa Medical and its officers, directors, employees, agents, licensors, suppliers, and platform partners from and against any and all claims, liabilities, damages, judgments, awards, losses, costs, expenses, and fees (including reasonable attorneys' fees) arising out of or relating to:
We reserve the right, at our own expense, to assume the exclusive defense and control of any matter otherwise subject to indemnification by you, in which event you will cooperate with us in asserting any available defenses.

If a dispute cannot be resolved informally, you and Truventa Medical agree to resolve any and all disputes, claims, or controversies arising out of or relating to these Terms or your use of the Services through binding individual arbitration, rather than in court, except as provided below. The arbitration shall be administered by the American Arbitration Association (AAA) under its Consumer Arbitration Rules.
YOU AND TRUVENTA MEDICAL AGREE THAT EACH MAY BRING CLAIMS AGAINST THE OTHER ONLY IN YOUR OR OUR INDIVIDUAL CAPACITY AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED CLASS OR REPRESENTATIVE ACTION. Unless both you and Truventa Medical agree otherwise, the arbitrator may not consolidate more than one person's claims, and may not otherwise preside over any form of a representative or class proceeding.
Notwithstanding the above, either party may seek emergency injunctive relief from a court of competent jurisdiction to prevent irreparable harm pending the outcome of arbitration.
