1. Overview

This Informed Consent document explains the nature of telehealth services provided through the Truventa Medical platform, the risks and benefits involved, your responsibilities as a patient, and your rights regarding treatment decisions.

By proceeding with a consultation through Truventa Medical, you acknowledge that you have read, understand, and agree to the terms described in this document. Your licensed healthcare provider will also review key consent elements during your consultation.

2. Telehealth Consultation Process

Telehealth allows you to receive healthcare services remotely through electronic communication technologies. Through the Truventa Medical platform, consultations may be conducted via:

  • Asynchronous messaging: You complete a detailed health intake form that is reviewed by a licensed provider
  • Secure video consultations: Real-time face-to-face visits with your provider via encrypted video
  • Phone consultations: Voice-only consultations when appropriate
  • Secure messaging: Follow-up communication through our HIPAA-compliant platform

Your provider will review your medical history, current symptoms, medications, and any relevant lab work before making clinical decisions about your care plan.

3. Risks & Benefits of Telehealth

Benefits

  • Convenient access to licensed healthcare providers from home
  • Reduced travel time and waiting room exposure
  • Access to specialized care that may not be locally available
  • Ongoing care management and medication adjustments
  • Secure, private consultations

Risks & Limitations

  • Technology limitations may affect the quality of communication (poor connection, audio/video issues)
  • Your provider cannot perform a physical examination, which may limit diagnostic accuracy
  • In rare cases, information transmitted may be insufficient to allow for appropriate medical decision-making
  • Delays in evaluation or treatment may occur due to technology failures
  • Security protocols reduce but do not eliminate the risk of data breach

Important: Telehealth is not appropriate for all medical conditions. If your provider determines that an in-person evaluation is necessary, they will refer you to an appropriate local healthcare facility.

4. Patient Responsibilities

As a patient using telehealth services through Truventa Medical, you agree to:

  • Provide accurate health information: Disclose your complete medical history, current medications, allergies, and any other relevant health information truthfully and completely
  • Update your information: Promptly notify your provider of any changes in your health status, medications, or personal information
  • Follow treatment plans: Take prescribed medications as directed and follow your provider's recommendations
  • Report adverse effects: Immediately inform your provider of any side effects, adverse reactions, or worsening symptoms
  • Attend follow-ups: Complete scheduled follow-up appointments and lab work as recommended by your provider
  • Ensure a suitable environment: Participate in telehealth consultations from a private, safe location with adequate internet connectivity

5. Medication Risks

If your provider prescribes medication as part of your treatment plan, you should be aware of the following:

General Medication Risks

  • All medications carry potential risks, including side effects, drug interactions, and allergic reactions
  • Your provider will discuss specific risks relevant to your prescribed treatment
  • Results vary by individual and are not guaranteed

Compounded Medications

⚠ Compounded Medication Notice

Some medications available through our platform are prepared by licensed compounding pharmacies (503A or 503B). Compounded medications are not FDA-approved but are legally manufactured under applicable FDA regulations by licensed pharmacies.

FDA-approved alternatives may be available. Your provider will discuss the appropriateness of compounded versus FDA-approved options during your consultation.

Off-Label Use

Your provider may prescribe medications for off-label uses when supported by clinical evidence and consistent with the standard of care. Off-label prescribing is a well-established and legal medical practice. Your provider will inform you if a medication is being prescribed off-label.

6. Privacy & Data Handling

Your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. We take the security of your personal and medical data seriously.

  • All telehealth consultations are conducted through HIPAA-compliant, encrypted platforms
  • Your medical records are stored securely and accessible only to authorized healthcare providers
  • We do not sell your personal health information

For complete details on how we collect, use, and protect your information, please review our Privacy Policy.

7. Right to Refuse Treatment

You have the right to refuse or withdraw consent for any treatment at any time. This includes:

  • The right to decline a prescribed medication or treatment plan
  • The right to seek a second opinion from another healthcare provider
  • The right to discontinue telehealth services and seek in-person care
  • The right to withdraw consent for telehealth consultations at any time

Refusing treatment will not affect your right to future care or access to our platform. However, your provider may document your decision in your medical record and discuss any associated risks of declining recommended treatment.

8. Emergency Disclaimer

🚨 IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 IMMEDIATELY

Telehealth services provided through Truventa Medical are not intended for emergency medical situations. Our platform is designed for non-emergency medical consultations and ongoing care management.

If you experience any of the following, call 911 or go to your nearest emergency room:

  • Chest pain, difficulty breathing, or signs of a heart attack or stroke
  • Severe allergic reactions (anaphylaxis)
  • Suicidal thoughts or self-harm ideation
  • Loss of consciousness or seizures
  • Any condition you believe requires immediate medical attention

Do not delay seeking emergency medical attention because of information you have read on this website or received through our platform.

9. Platform Role & Provider Independence

Corporate Practice of Medicine (CPOM) Compliance

Truventa Medical operates as a technology platform that connects patients with independent licensed healthcare providers through OpenLoop Health. Truventa Medical does not:

  • Practice medicine or provide medical advice
  • Employ or direct healthcare providers
  • Make clinical decisions regarding your care
  • Prescribe medications
